7/17/2023 EnchantaVaughn Approved 1. Behavior: Client appeared guarded and offensive due to encountering other challenges in the home. Client was inattentive and unfocused as it was challenging and difficult for her to remain in one area of the house. Client would periodically interrupt QMHP while she was reviewing important information related to her case. Client began to exhibit a temper and refused to do anything her counselor asked her to do. Client was defiant of QMHPs prompts 2-3 times. 2. Behavior: The client struggled to maintain self-control during parts of the session when she became distracted or unfocused. Client appeared unapologetic for participating in disruptive and violent actions that heavily affects everyone around her. Client’s demeanor then became very reserved and limited in her verbalizations and comments regarding the topic of discussion. Client stated that she feels more connected with her counselor than some of her loved ones. Client was supported with making changes at home. 1. Intervention: QMHP reviewed methods and tactics to help client positively advocate for herself by the use of assertive communication and conflict resolution skills daily and as well as to assist in reducing negative actions in the home. QMHP reviewed strategies targeted at helping client to work through her difficulties. QMHP offered new ways to heighten client awareness of her ill-mannered behaviors, as well as helping her with functioning more constructively within her daily life. 2. Intervention: QMHP facilitated an activity on emotions to assist client to be able to identify and positively express them without interfering with emotions she wants to keep private. QMHP directed client to write the words of thoughts and emotions that she feels when she thinks of her close relationships. QMHP discussed client’s responses with her and modeled for her how to approach her family/friends about her feelings, good or bad. QMHP role played with the client to assist her in bettering her social-emotional-mental health. 1. Response: Client expressed that when she is involved in a conflict she will take a self-time out. Client appeared disinterested in regard to the information that was being provided to her by the QMHP in response to how her negativity affects herself and others. Client acknowledged QMHP’s redirection after she repeated herself 2-3 times. Client demonstrated that she was able to process productive expressions of anger once she calms down. The client showed her gratitude for being able to release physical tension stating that she felt relieved. 2. Response: Client was attentive to the role play activity, even implementing strategies she just learned to reduce her negative responses. Client explored numerous ways to express her anger and negative behaviors during the role play with QMHP. Client demonstrated her knowledge through her provided examples. The client responded in a constructive manner acknowledging the affirmative reinforcement that the QMHP provided. Client understands that mother is trying to aid client with her prioritizing skills and create structure in the home.
